I would say Forma is one of the best dental offices in Miami. The office is amazing and the staff is so friendly. I have bad anxiety and they made me feel so comfortable. It seems like a brand new office, really cool design, new tech, art, equipment.... highly recommend Dr. Alves and Forma Dental.
