We will verify insurance benefits prior to your initial visit to the clinic. In order to serve you better, we have included a link on this page to the admission documents. After completing them in the comfort of your own home you can bring them in at the time of your first visit. We do file directly to your insurance carrier, however the patient is responsible for their co-payment, deductible, or non-covered amounts at the time of service. If there is a question of coverage, we will contact you to resolve any payment issues. We ask that you bring your referral or prescription with you on your first visit.
